N vs. R vs. X

This has probably been covered before, but I was just curious where the dividing line between the different rankings is. Usually they seem pretty consistent (N=nude without anything "gynecological", R=nude with gynecological shots, X=penetration). But sometimes they seem pretty far off, like "Haunted," recently posted by calibre. It got an X, but it seemed pretty R to me, unless I missed something.

Just wondering. Maybe it's nothing more than clicking the wrong box on the ol' CMS before uploading, huh?

N means Nice
R means Really nice
X means eXtra nice
